(a) At least once each year, the licensing agency will monitor an assisted living home or will require the home to submit a self-monitoring report to the licensing agency on a form supplied by the agency. The annual report must contain the information requested by the agency, to show that the home continues to comply with AS 47.33 and
this chapter.

(b) The failure to submit an annual report as required by the licensing agency, or the submission of a false report, is grounds for issuance of a notice of violation under AS 47.33.530.

(c) The licensing agency may accept the report of a third-party, independent evaluation of an assisted living home, made on behalf of the home, if the report contains substantially the same information required by the licensing agency under this section.
