According to record review and interview, the medical examination report for one of three residents sampled whose charts were reviewed (#1) did not address all required elements.
Findings include:
Resident record review during the – survey revealed that resident #1 was admitted to the facility on (date). Further review of this individual’s file found that on the latest health assessment form . No diet order had been designated by the examining health care professional. in addition. no diet order could be found throughout the resident’s record. interview with facility administration at approximately 11:30am. on (date) verified that said order was unavailable for inspection.
